The Transformation Manager delivers strategic change programmes, to improve the quality, efficiency, and sustainability of services across the organisation.
Working collaboratively with clinical, operational, and corporate teams, the postholder ensures that change initiatives align with organisational priorities and deliver measurable benefits for patients, staff, partners and stakeholders.
The post holder will be required to work flexibly across the primary care/secondary care teams as required, based on local priorities.
Main Duties Programme Management Support the planning, design, and delivery of complex transformation projects across services.
Develop project plans, business cases, and benefits realisation plans.
Manage project governance and risk management in line with agreed budgets Stakeholder Engagement Build strong relationships with internal and external stakeholders including clinicians, managers, commissioners, and service users across Evergreen Life and Omnes Healthcare Facilitate co-design workshops, engagement sessions, and regular communications to ensure inclusive and informed change processes.
