Job Summary The Population Health Coordinator role is designed for individuals who possess a passion for and a commitment to helping underserved populations access to important preventative healthcare services while maintaining HIPAA compliance standards.
The coordinator conducts outreach, participates in workgroups, and tracks productivity and program results.
