The Housing Support Specialist is a highly engaged and mission-driven role that provides one-on-one support services to navigate housing opportunities, locations, and overcoming barriers with intentional, individualized service planning for adults with intellectual and developmental disabilities (IDD).

Overview The Housing Support Specialist promotes and coordinates housing stability activities to individuals with intellectual and developmental disabilities.
The specialist coordinates with clients, landlords, and local community resources to develop housing plans and deliver independent living skills that promotes housing retention.
Client Support and Case Management Complete intake assessments for new or progressing clients who are referred for housing services, ensuring all referral documentation is collected, reviewed, and validated for program enrollment Maintain an active caseload to provide one-on-one support in identifying community resources, developing housing plans, and facilitating referrals and linkages as indicated Conduct comprehensive assessments across all domains of a client’s life to determine service needs and priorities Provide case management, including coordination and monitoring of person-centered plans (PCPs), and ongoing progress evaluation Offer individual supportive counseling and assist clients with symptom management, crisis stabilization, and goal attainment Educate and support clients’ families, advocating for client rights, preferences, and self-determination Housing Services and Development Navigate housing assistance programs including Section 8, HUD programs, HOPWA, and state-specific IDD housing initiatives Conduct pre-inspections for new unit move-ins and relocations; coordinate with housing inspectors to ensure compliance with program and safety standards Obtain and document results of housing inspections and ensure all required follow-up actions are completed Develop and identify housing opportunities within the community; maintain updated knowledge of available housing resources Serve as a liaison and advocate between clients, landlords, and community partners to facilitate communication and problem resolution Program Implementation and Coordination Organize and conduct structured group and individual activities that support clients’ housing goals, community integration, and independent living skills Provide direct service interventions aligned with the client’s PCP and in coordination with the multidisciplinary team Perform field-based outreach and support—approximately 80% of work performed in community settings Documentation, Reporting, and Compliance Maintain comprehensive documentation in compliance with agency policies and regulatory requirements, including housing plans, progress notes, person-centered planning (PCP) records, and all Medicaid, state, and federal documentation standards Conduct program compliance activities including audits, evaluations, and reporting to ensure adherence to program, agency, and funding source requirements Manage annual housing recertification process by obtaining, verifying, and maintaining all required documentation to ensure continued eligibility and compliance Administer financial management services for clients in the representative payee program, including preparation and oversight of monthly budgets Drive program excellence through active participation in annual service goal development, implementation tracking, and progress reporting against established metrics Collaboration and Community Engagement Identify housing resource materials and procure them by agency procedures Represent the agency in community meetings and collaborative initiatives focused on housing and mental health services Coordinate transportation to clients for community meetings, inspections, or other necessary appointments Provide on-call crisis intervention support during evening and weekend hours as needed Education Minimum an Associate degree with 5+ years of relevant experience in IDD services, case management, or housing support Experience Experience with knowledge of affordable housing/rental assistance programs, landlord/tenant laws, Tax Credit Programs Knowledge of the (DDA) system, eligibility requirements, and related funding mechanisms Background in providing on-site job coaching, workplace accommodations, and employment retention support is preferred Certifications & Licenses Valid Maryland driver’s license with a clean driving record and reliable transportation Must have the DDA state approved certificate from Maryland Inclusive Housing (MIH) Compensation & Core Benefits Annual Salary: $60,000 – 70,000 Comprehensive health, dental, and vision insurance Retirement plan Paid time off and holiday pay Mileage Reimbursement Flexible scheduling to balance housing services responsibilities Preferred Additional Qualifications Bilingual capabilities Background in disability advocacy or services Experience with assistive technology and communication devices Knowledge of evidence-based practices in skill development and community integration Allied Community Care Inc.
